What exactly makes up the packet, and what can go wrong with it--I don't want to do it wrong and have it become rejected...

I'm wondering.. what comprises "the Packet?" Is it the letters (family and individual), the contact summaries, the psych eval, physician letter, photos, and responses to questions about MGB? Any suggestions on what I can do to ensure it's got everything it needs so I can have this surgery.. wish I wasn't in school, or I'd apply to have it done tomorrow.. but alas.. all good things in time. Hi everyone: I've already made the decision to have the MGB... but my primary provider is a NP. I need an MD to support me and agree to follow me in Anchorage or surrounding area...does anyone have any leads on someone? I travel to the remote radar sites sometimes and they are military so my internet access limits some of the sites I can go to. Nice to be home now. I was out on the coast and the snow was flying and cold. It's between 0 and 20 here in Wasilla with a high Barometer and clear skies. Plenty of snow so it's real pretty. My surgery is only a couple of weeks away now, we're leaving for Nevada the Sunday after Thanksgiving. Even my husband is excited about the whole thing. It seems almost magical that something can be done to help the obesity. To say that I have been diligent in my efforts to keep the weight off over the years would be an understatement and still it got away from me. The last month before the surgery seems to require a lot of organization. I completed the essay portion and the personal contacts early on. I also got the psychologist taken care of. My insurance wanted six months of weight loss follow up just prior to the surgery,letters from doctors and they needed three weeks to evaluate. Dr Rutledge wants the packet in before two weeks prior but the physical, labs and EKG have to be no more than thirty days out. It takes time to arrange leave from the office, impossible over the holidays. Thanksgiving slows up everythng . . . etc etc etc. It can be done, however. The way to get the weight ticker is to go to the website that offers it - it's on a logo on the ticker - and follow the directions. Mostly its just cut and paste stuff. It took me a while to realize I had to exit out of my web page and reload it to get it to take. It's probably best to start with the essay portion of the packet. By the time you have completed that you will have a greater understanding of the surgery and that makes everything else easier.
